OptinMonster Custom HTML with Marketo (or any popup tool alternative?)

I'm using OptinMonster integration with Marketo, which only allows showing 3 fields - Email, Name and Phone.

I'd like to use a different field and therefore I need to use their Custom HTML Integration. However, for using this I'm required to supply form HTML which I can't find anywhere... (I can only see the embed code)

Anyone knows if that can be solved?

If not, which other popup tools are you using and can recommend of?

The Forms 2.0 embed code creates a functioning HTML <form> tag; it uses JavaScript to create and manage the form and its inputs, but the underlying elements are still there. Inspect the element (in F12/Developer Tools). You also, IIRC, can insert the embed code itself as the Custom HTML.
