No records found using Fills Out Form Filter

Hi,

I have facing an issue, so I created a Smart campaign to capture who ever fills out the form using trigger and in the result tab its giving me 5 records those have filled the form.

When I am using Filled Out Filter tab using a smart list it gives me 0 records. I am not able to figure out what the issue might be. Can any one help me over here.
