Missing Comment Field

I am missing the comment field after a couple of test forms completion passes.  I have progressive profiling enabled but I have the comment box unchecked, yet when I go to fill the form out for the 3rd time of testing, the comment box is missing now from the form on the landing page.  Any ideas as to why this would happen?

I would clear your cache & cookies completely and/or try in different browsers to make sure this isn't happening to just you. If that doesn't help, it might be a good idea to create a ticket with support as they can get into your instance and see what you're seeing where none of us really can.
