Marketo/LivePerson Integration- Survey Script

I am trying to integrate Marketo with LivePerson. After reaching out to LivePerson, they provided me with a document with steps to achieve this. However, I came across this step (see picture below) which asks for a Marketo Script. 

Can anyone assist with where I get this script? Do I create a form on Marketo for this?

What you want to do is make a Custom HTML block on your landing page and copy-paste this script in there, and then change the values as the instructions say.
