Marketo embedded form checksum missing error

Hi, running into an issue with using the embedded Marketo form. I am using the correct <script> tag but it keeps leading me to a "checksum missing" 400 error. It seems like there is a backend functionality that is messed up somewhere that might be more from the Marketo side than our codebase. 

Any solutions?
