Looking for an online assessment vendor!

I'm looking to build an online assessment tool, sort of like a survey that outputs a score at the end. I'm looking for a vendor that can create this type of interactive page -- has anybody in the community built one before? I'm looking to have it integrate with Marketo as well.

After looking into this further, I know exactly what I'm looking for now. On other websites, they're known as personality quizzes. Here's an example: http://www.selectica.com/quiz or http://www.animalinyou.com/test.php

I haven't been successful finding a vendor or an online service that can generate this (that also generates leads!)
