Light Box form dismissed after submission

Hello,

     I am trying to have a form load in a lightbox and then be ignored upon submission and reload of the page. Is this possible? I found some script snippet to add for this but it is not working. Can anyone help?

What script did you use for the ignore upon submission and reload function?
