Lead Source is {{my.source}}

I have numerous leads coming through with a lead source as Lead Source = {{my.Source}}. 

This is happening with most of our leads even though the actual lead source should be web or something similar. Any ideas on why this could be happening? Is it a setting I have missed? Any guidance would be greatly appreciated.

You probably have that token as the default value for a hidden field, but you're using an embedded form (which can't use tokens).

    I faced the same a issue a few days ago. One of my team mate put the token as the default value in the form hidden field. Just go to one of the lead with this source and look at the activity log. You should be able to see the source of this value.
