Lead Flow settings for forms with multiple choices???

Does anyone know how to add "Lead Source Descriptions" to a flow when there are 4 or 5 choices on the drop down.  I want to be able to track which choice they want us to contact them about.  For example"

Our contact form has a field that asks - what can we help you with?  There are 6 choices and I would like to be able to track which choice they made via a flow.  Would it be

If Lead Source = Contact Us

Attribute = Lead Source Description and then the choice

and so on for each choice that exists on the drop down menu????  Or will the flow just take the first one listed??
