Lead completed form with out filling out mandatory email field. How?

I have a lead who filled out a form with a mandatory Email field- and somehow still became a lead with out completing the email section.  How can this happen?  Any ideas? How can we prevent it from happening again?

Kate,

A good place to start would be the activity log of the lead. Could be something with the form, got cleared, etc.

You could even paste the log here and we can help you figure it out.

Charlie - thanks - I tested out the LP myself - put my email in -and I came across as a new lead  - no email came through.  The form would not let me advance when I was entering the information - but my email did not pull through to my lead profile.
