Thanks, Scott! The only downside to SurveyMonkey is if your job didn't fit into those criteria, and you typed in something for other, it still forced you to pick a function above.

Did it. As Dory mentioned the job choice forced me to select a job type. There should probably be a a radio button for "other".

Kristen - Please take a look at our LaunchPoint partner called SnapApp. It is an integrated solution that allows you to create interactive content including surveys, and then update lead record with desired info after completion.
