JS Querystring Filter fail?

I have one URL with 3 querystrings, which I want to filter (each has it's own Smart Campaign) before sending an autoresponder email, however this is not working and the lead receives 3 emails, running through each campaign having only completed one form on one of the Landing pages.

Strangely, this has worked for us in the past (this is a cloned campaign).

Can anyone offer any pointers or advice please?

Please post a screenshot of the SC Smart List and a screenshot of the Lead Activity Detail for the activity that unexpectedly triggered the SC.

(What does this have to do with JavaScript, though?)

Thanks.  This was an issue with 3 filters, which was resolved by removing two in each case.

Seemed to have worked before but now threw up errors, however has been solved.
