InputMask plugin breaks when there are 2 forms on one external page

Has anyone had an issue with embedding 2 forms on an external webpage and one of the forms includes the Phone Number input mask plugin but that plugin breaks because there are 2 embedded forms on the page? I've made many attempts to try fix the issue. And I haven't found a resolution online.

Any helps is appreciated.

There's a form in the main content area and the 2nd form is an email input field in the footer area.

Load your form #1250 (the email-only form) before #1133 and you'll work around this. There's a bug here but I won't get into the deeper solution.
