How do I set completion actions for a Form? Such as, "Send notification email and form record to name@email.com "

Hey Jordan, you would create a smart campaign to do this. The smart list would be triggered by "fills out form", and you can constrain it by page if you want to. The flow step would be send alert email (you will need to create an alert email for this purpose), and you can then choose who it would be sent to.
