Help with Form 2.0 embed on our site

My front end developer is having a really difficult time getting our contact form to look right on our site. I'm using the embed code. It looks like this: CRF Health - Contact 

Notice the message box is incorrectly styled, off a bit on the left side, and not as wide as it is set in the form settings.

When we use the code found on the community to generate a marketo-css "CLEANED" form, we struggle to replicate the look of several smaller fields and one larger message field, since all have the same ID.

Anyone have a sample site up with a marketo form embed playing nice?
