Has anyone tried to run a native short survey using only Marketo form fields?

I am planning a 3-question survey with up to 5 radio button responses for each. Has anyone had the opportunity to set up a survey using only Marketo form fields? Any guidance would be appreciated.

Yep - it's fairly simple. You just create custom fields in Marketo for each of the questions you want answered, set up a form as the survey, and have the data flow into those fields.
