Has anyone created a form with a Primary and Secondary contacts?

We have a team that is trying to capture the name of a primary and secondary contact within the same form?  Has anyone tried this?  Can it be done?

The issue with current forms in this sense is that you end up tying multiple emails to an IP address skewing your results and tracking efforts. We are all looking forward to what Marketo's Form 2.0 will allow us to do that might enable such a thing. Others may have a better around until then.
