Getting form Fields to my Customer Success Team

We have a demo requests on our website and are using marketo forms to capture info. That triggers 2 events.  Lead Lifecycle scores it (fine there) and starts the sales process. But I also need customer success to create them a log in into our PIM.

Ideally, I need the actually field info from the form to be IN the email alert that goes to customer success.

Is this possible? Looks like the triggers that I can pull into the email are not from the form.
