I have seen instances where folks have gotten creative and used images of radio buttons in emails that basically served as a form. This will allow you to bypass the traditional no-no of putting forms in emails as Murtza alluded to.

I would recommend just using landing pages. You could insert a form in an email, but it is not a best practice and a possible security risk.
