FORMS: Company Required, Showing Up "Unknown" on Field

Hi

I have a required company field on my forms but its showing up as "Unknown" when a Lead fills out the form. I do have progressive profiling enabled is that the reason this could be happening? Is it as simple as fixing the way the profiling fields are positioned? Attached a screen shot of form

Need a little more info and please move to Products​ to continue. (Move link is to the right.)

After moving, pls provide a URL. And also, yes, if you have Company as a ProgPro field that is definitely going to impact its required status. By definition a ProgPro field that is not currently shown cannot be required.
