Form with 'Select One' field needs help

I have a form with a required "Select one..." field and two other "Select one..." fields. When I preview the form it looks as it should. When I test the Landing Page wiht the form only one of the "Select one..." fields shows correctly.The words "Select One..." do not appear on 2 of the 3 fields.  Any ideas on how to fix?

Is "Select One..." in your value list within the field when working within the form builder?

Yes, Select One... is in the value list within the field, however, the Select One... randomly appears when testing the landing page.

I believe the problem was in the testing to the same leads over and over. The original issue did not occur when going out to new leads.
