Form Submission Smart List/Reporting based on Web Page "Contains" logic

I was wondering if it's possible to generate a smart list based on whether a lead filled out a form on a certain URL criteria. 

For example, I would like to see how many leads have fillled out my "Resource Download" form on my Canadian & US sites.

Unfortunately, this doesn't appear to be an option in the smart list tool. Could anyone offer any suggestions/workarounds for how I could achieve these results by some other means within Marketo? Perhaps through the reporting tool?
