In sum, use a Rich Text widget on the form that includes the disclaimer + checkbox, then follow my directions.

Only thing is we need the disclaimer button submit button (as it's quite long) is there a way to add it?

I don't know what you mean by "the disclaimer button submit button." Do you mean you want the disclaimer on a button?
