Form Fills Not Being Captured

Hi,

I was testing a form out and strangely enough some of our form fills were being captured and some were not.

I noticed that this was specifically in the case when a new lead that does not exist in the database fills out a form, they will neither be captured nor created.

Has anyone else experienced this? Does anyone know how to fix this?
