Form field that allows a PDF to be uploaded and submitted

We have a form that needs to have the capabilities to upload a PDF. The screen shot below isn't mine, but the Profile Image would be  - Upload your X (pdf) and the Browse would allow the user to browse to the PDF and then "Get Started!" would be "Submit".

Any ideas on how to do this in a Marketo form?
