Form field not converting to alert email

I set up a form that has a date field in which the person filling it out the form can choose from a drop down of date options. The purpose of this is for meeting scheduling, the person filling out the form can pick from some options we provided. When the form is filled out we have set up an email alert. The form displays correctly in marketo and on our landing page, but when we receive the email alert the date field does not populate the information chosen during form fill out. Additionally, when we export the form data out of marketo no date information is available. The other fields are coming through but this one is not. Can someone help us figure out why this might be?
