Form details not filling in

Here is the scenario:

I use the same form for our contact us page on our website and for content downloads on a landing page.  When I use the form on our webpage all the fields populate in marketo, but when I use the landing page form they do not.

Any thought here as to what can cause this?

is that a Marketo form or an external form?
 
Marketo blocks any attempt to post directly from external forms. The appropriate methods are SOAP API's syncLead or Munchkin API associateLead.
 
On the other hand a landing page may fail to post if JavaScript or jQuery fail.
 
Could you share the link to the contact us page on your website and a landing page?
I can follow up via Support Case if that is not feasible for any reason.

This is the link where I can collect the company type:

brandlift.vizu.com/about/contact/

This is an example of a form which I cannnot collect the company type:  

go.brandlift.com/ANAIndustrySurvey.html

They are both Maketo forms, the same exact form.
