Form 2.0 duplicate

Hello
I created only ONE form 2.0, but once approve draft, there are same two form.

Can you clone that page and remove the form then approve and tell us the URL for that page?

The html looks like ther emight be 2 forms on the page, 1 form 2.0 and another coded into the template.
