Field with visibility rule not appearing on embedded marketo form

I am using an embedded Marketo form on our wordpress website www.signanthealth.com/contact-us. There is a visibility rule that says to display the state field if the country field chosen is United States. I've tested the form in preview and on a Marketo landing page and the field appears as expected. However, the field does not display on the website and I can't figure out why. 

I don't have progressive profiling or form pre-fill enabled on the form. I saw that referenced in some other articles.

Any suggestions would be greatly appreciated as the field is critical for us.
