Failed to update/create lead during syncMultipleLeads soap request

Hi, I am sending a syncMultipleLeads request in soap and on some occasions it fails to create/update lead
but no exception occurs.
What might cause a failure on syncing a lead?

It might be a case of duplicate leads, but it is hard to say without seeing the specific requests.
