Exporting information filled out in form

We would like to be able to export all data that someone inputs when filling out a form, including in a "Comment"-type box (text field)...What is the best way to set this up?

Hi Laura,

Create a Smart Campaign with a filter for "Filled Out Form." On the Schedule tab, click the number of leads associated and edit your view to contain the fields that are on the form. From there, you can export an Excel spreadsheet.
