hi all. we've embedded a global form across various pages on our website (aka not marketo landing pages) and want to run a report that enables us to see what form fills have come from particular pages. what's the best way of doing this?

There's no report type in Marketo that treats Filled Out Form as the pivot point, so you can't generate this from the UI. You will find that all reports involving FOF use the person, not the form or the hosting web page, as the pivot (i.e. as the data in the row).

Exporting the activity logs via Bulk Export API will give you this data surprisingly easily, it's just not a canned report type.
