Dynamic Country Province Pick List feature stopped working

I followed this to the letter: https://community.marketo.com/MarketoArticle?id=kA050000000KysHCAS, I see that the state field disappears, but choosing Canada does not make it reappear. This worked correctly previously and the United States still works correctly. Also, I do not have progressive profiling enabled.

Hey Tom, I can't see the article that you're linking to, but it sounds like it might be a problem with the visibility of the field, if you cannot see it when selecting Canada.

Maybe if you could add a screenshot so we can see the setup?

Nothing  has changed on our end, as far as configuration, it just suddenly stopped working when you select Canada.

You should use Forms 2.0 which supercedes the document mentioned.

Agreed, Forms 2.0 allows for this much more easily, and without issues such as this.
