Duplicate Leads Not Showing up in "Possible Duplicates" List

We are experiencing an issue with our leads database, where Marketo is not catching all of our duplicate leads and putting them in the "Possible Duplicates" list. These leads have the same values for the first and last name fields, but all of the other information differs for these leads. 

Just a sidenote that one of these duplicates may be created by someone on our team directly into Marketo. Could this be why Marketo is not flagging these two leads as duplicates?

For leads to be in Possible Duplicates, they need to have the same email address.

This is the only matching that Marketo does. If the first and last name are the same and one is missing the email address, Marketo will not recognize this as a duplicate.
