Does marketo unsubscribe the email address or the record?
If I have duplicates with the same email address and the person unsubscribes does the email address get unsubbed or just that record?

Marketo has what we call durable unsubscribe. If an email addrss unsubscribes we check your database for duplicate email addresses, if we find one it will be auto-unsubscribed
