Distinguish between multiple custom objects

We have a custom object set up for active customer subscriptions (pulling down from our CRM). A customer can be subscribed to more than one product, and we have hundreds of different subscriptions.

Is there a way (via a smart list), that I can distinguish whether a lead has just one subscription/item or multiple subscriptions/items within that custom object?
