Demandbase + Marketo forms

Has anyone worked on integrating Marketo forms with Demandbase successfully? We're trying to integrate Demandbase with our website and Marketo forms and are experiencing issues passing information from Demandbase to Marketo. Would love any documentation on how to best do this if anyone has any! Unfortunately, Demandbase has been pretty unhelpful thus far.

What kind of issues exactly?

Why have they not been helpful?

We've added the hidden fields they requested us to add and the tag, but none of the information is populating for the leads.

When we asked them for examples who have done this specifically or to look at the actual landing page set-up, they haven't been able to debug the issue beyond telling us, "just make sure you have all the hidden fields". We've asked them to connect us with reference customers who are using the integration with Marketo, but haven't heard back  yet. Hence why I'm reaching out to see if anyone is actually using the Demandbase integration with Marketo forms.

We've had a completely opposite experience in working with DemandBase.  We use them extensively for advanced analytics, targeted advertising, Sitecore integration and Marketo forms.  Any time we had issues with the forms integration, we got assistance that same day.  If it helps, here's one of the forms that has the DemandBase integration included: Avanade Contact Information & Office Directory | Avanade

@Dan Stevens​, this is super helpful! Would you happen to be able to share a screenshot of your form editor and how the hidden fields look added there in Marketo?

We didn't need to do anything to our forms (except include a hidden custom field for those forms where we didn't want the DB integration).  Once the DB script is contained on your pages - and you've mapped all of the DB fields to your Marketo fields (using the Excel template provided) - everything is administered on the DB side.
