Date of Activity is exactly X days ago and since then

Is there a way to set a filter for exactly X days ago without excluding leads that may have had that same activity since then?

For example:  Return all leads that had a data value change 5 days ago, but do not exclude leads that may have had a data value change within the last 4 days

No, the filter needs to be dynamic so using actual dates will not work.

Seems like in the past we have used a standard salesforce filed to do this that syncs down to Marketo. would that work?

Salesforce workflows are probably your best bet for this exercise.
 

If you could provide some insight into use case, perhaps we could suggest alternatives.

Nope.  Please read "without excluding leads that may have had that same activity since then?"

Also read "but do not exclude leads that may have had a data value change within the last 4 days"

Salesforce workflows will not work because we are trying to create triggers when certain activities are logged in Marketo.  Not just when data is changed but any activity such as web visits.
