Creating a form for a survey

I created a registration form for a live event that includes a survey. The survey has multiple options and I used checked boxes. When I added the field for the survey I was prompted to give a Field Type. I couldn't find a field type that matched what I wanted. Is there a way to create a field type?

Hi Kara,

To create a field, you will have to go to Admin> field management > and click on new custom field.
