Controlling unapproved or approved with draft email scheduling?

It looks approved with draft emails can be scheduled and sent although it doesn't include the changes, sends the original approved version, correct?  Can unapproved  emails be sent?  Is there a way to prevent either from happening?

Correct, the system will always send out the approved version of an email if there is a draft.  Unapproved emails cannot be sent.  The system will not allow you to select an unapproved email and you cannot unapprove an email if it is used by a campaign.

Have there been cases where an approved email was changed to approved with draft by the system...even inadvertently?

If you edit the template that the email is using, it will make every email using that template a draft.  You will then need to go in approve them.  This can be done in mass on the email summary page in the Design Studio.
