Contact Us Form Alert Email only sending/delivering once

Okay so I created a form within Marketo and then using a smart campaign activated trigger set it up to send an email alert to a series of individuals based on the region selected within the form eg.

If form field 'Region' was Asia Pacific > send an email to xyz@xyz.com.

The problem doesn't lie in the initial process as it worked seamlessly. The problem occurs when the same individual completes the same form multiple times, potentially with different requests/messages/enquiries. The alert doesn't get sent at all or at least it doesnt arrive so I am not sure it even gets sent. I have tried it with a variety of different test email addresses and in every case it arrives the first time but no more thereafter.

I am probably missing something simple or need to change a setting someweher but any help on this would be greatly appreciated.

On the "Schedule" tab of the Smart Campaign, make sure you have leads able to go through the flow every time:
