Can different forms be used on one landing page based on who visits?

I am building out an email preference page, and depending on contact (lead) type, they will be given different emails titles to opt into. Eg. I've created a 'loyalty' form that provides options for the regular email titles plus our loyalty newsletter. And a 'regular' form for those who have not yet earned loyalty status. Can I use the same landing page for all but have the specific form for each contact type pulled in? Thanks!
