Boolean form answers visible in alert email

I am tracking a campaign that needs special alerts. Let me explain a few details. The form is on a non-marketo site. They client is given a custom link. They go to the website and click though to a page where they are able to select three options (set at boolean in marketo): request meeting, download white paper, or download infographic. The form is embedded on their website and gets automatically submitted when they click any of the three options. I need to set up an alert email that states which one they chose so I can figure out how to handle the follow up. Please help!
