I've searched high and low and don't believe this is possible, but before giving up, thought I'd ask. Is there any way that anyone's found - roundabout or not - to create a lead and associate it with a program in Marketo via their REST API?

What do you mean by "associate with a program" ?
Add it as a program member ? setting the program as the acquisiton program ?
In both case, the simplest way is to request a campaign for the lead, and have the campaign do the job. Use Request Campaign » Marketo Developers
