Adding people to named account - ABM Tool

I am trying to add pipeline to a named account in Marketo's ABM tool but I am not having success.

First I created a smart list of leads and then tried to add them to a named account - but it only added 3 out of 10 that I tried to add.

Second I created a smart campaign to add to a named account and again not all the leads where added.

What am I missing? Does a lead need to have a certain criteria before it can be added to a named account?

We can only change account assignments for non-CRM people. Can you provide more details around the type of people you are trying to add to the Account?  We need more information in order to determine whether or not the tool is behaving correctly.
