Add activity to Salesforce contact?

We'd like to add an activity to the Salesforce contact that triggers when a data change is identified in a certain field.  I'd like that activity to include tokens for this field and another field. Is there any way to do this that just goes straight to the activity log?  

I'm aware the Interesting Moments/Sales Insight has funtionality to do this, but I am looking for a way to go directly to the contact activity.

You first select SFDC Type=Contact in your smart list.

Then use Create Task and put the tokens in the fields there.
