Abandoned Form Report

I'm trying to create a report showing the number of times a page with a form was accessed and how many times the form was filled out.  Following the instructions I've found, I've created a Landing Page report but it doesn't give me the ability to filter out Thank You pages or pages without forms.  

How do I create a report showing only pages with forms, how many times they were accessed and how many times the form was filled out?

You will need to include only the pages you want using the Setup tool that lets you go through the Marketing Activities or Design Studio tree.
