Landing page for an Email Opt-out , Subscription list

I am trying to fix a pre-existing opt-out landing page for our subscribers to cancel their email subscriptions, but I can't figure out how the,

"Subscription list", was added to the landing page. We are getting an error message, saying that we have duplicate Subscription list.

@georgevi2 By looking at the subscription list names from the screenshot there don't seem to be any duplicates. Would it be possible to see the form field mapping within the email designer where you have selected the subscription list for each option>?
