Quick check- In the form's field mapping / schema settings, confirm the field you're using as the identity is mapped to the same namespace the profile actually uses.

A mismatch here is the most common reason a submission ends up stitched to the wrong profile.

Support ticket might help get a quick turnaround on this one.
