If we are upgrading from AEM 6 (sp3 to be specific) to AEM 6.4, should we be running 'Lazy content migration'? or is it only applicable when upgrading from 6.3?

Lazy content migration is relevant starting with versions 5.x, so the answer is yes - you need to run it for 6.0 as well.

By looking at Adobe Experience Manager Help | Lazy Content Migration , it seems like for 6.0 is immediate.
